To Complete Registration for 2018-2019:

1. Pay one registration fee per family.                                                                                                       Early Bird registration is $25.                                                                                                                Write registration check to Lakeview/LEEP.  

2. 1) Print, read and sign Parental Agreement form.                                                                                 2) Print, fill out, sign and notarize the Ministry Activity Participation form. 

3. In order for your child to be registered ALL FORMS, including Medical Release Form and Parental Agreement Form MUST be turned in with ALL FEES.                                                              *If you only pay for 1st semester, another registration fee will be due with 2nd semester tuition.                                                                                                                                                                  *If you pay for the year, you only need to pay one registration fee.

4.
